Print this form in LANDSCAPE mode with around .3" margins

Font settings used in Netscape were:
  • Variable width font: Tahoma Size: 8
  • Fixed width font: Lucida Console Size: 9
Although probably the only change you may have to make
is to shrink your font sizes down to get the page to fit on the
LANDSCAPE page. Be sure to go into File, Print, Properties
and set to printer to print in Landscape mode.
Then print from that screen.
